What is Remote Therapeutic Monitoring?
Remote Therapeutic Monitoring (RTM), not to be confused with Remote Physiological Monitoring (RPM), is the collection of therapeutic, non-physiologic data, collected via a medical device or software, as defined by the FDA. Such data can include signs, symptoms and functions of a therapeutic response related to respiratory system status, musculoskeletal system status, therapy adherence, and therapy response.
How Do Remote Therapeutic Monitoring and Remote Patient Monitoring Differ?
RTM and RPM differ in three main areas: 1. The type of data collected – therapeutic, non-physiologic data vs. physiological data, 2. The manner with which the data is collected – RTM data can be self-reported while RPM data is digitally recorded and transmitted, 3. The ability to bill for such services – non-physician providers such as PTs, OTs and PTAs, are eligible to bill Medicare directly for RTM services.

With RTM, physicians can monitor patient medication adherence and thus improve outcomes. FAQ’s
When are the RTM codes available to be billed directly by Physical Therapists?
The RTM codes were added to the CMS Fee Schedule for Reimbursement on January 1, 2022 and can be billed immediately and directly by PT using their own NPI number.
Does Medicaid or private commercial payers follow Medicare and reimburse for RTM?
We are unaware of any private payers or Medicaid programs that are reimbursing yet for RTM. However, similar to when RPM codes were introduced, it is common practice that these payers will follow the guidance of Medicare and begin reimbursing soon for RTM.
What does direct supervision mean and does that provider need to be a physician?
Direct supervision means the billing provider needs to be located in the same physical space as the Physical Therapist (PT), Occupational Therapist (OT), or Physical Therapist Assistant (PTA) who is treating the patient. However, during the PHE, direct supervision may be administered virtually, assuming they are immediately available while the PT/OT/PTA is providing the RTM services. Non-Physician Providers such as PT’s/OT’s/PTA’s who are eligible to bill Medicare directly for their services and whose scope of practice includes RTM services are eligible to bill for RTM and serve as the supervising practitioner.
What type of devices can be used to bill RTM codes?
A device must be FDA registered/cleared, per their definition of a “medical device”. RTM data can be automatically transmitted through software as a medical device (SaMD) or reported by the patient.
How many days per month are required to collect patient data?
16 days of data needs to be collected per every 30 day billing cycle.
Can RTM and RPM codes be billed simultaneously?
As for now, RTM and RPM cannot be billed for the same patient during the same calendar month.
Are the RTM codes subject to patient co-pays?
Yes, the RTM codes have a patient responsibility component. However, during the current Public Health Emergency (PHE), providers have discretion as to whether or not they charge patients for the copay.
